Service Description

• The purpose of this assessment is to determine the extent to which the injuries affected the level of function, independency in activities of daily living (which include personal management, social ability, leisure and occupation/academic work ability and capacity), and to discuss any additional assistance, specialized equipment and devices and adaptations that may be necessary. • To ascertain the extent of injuries sustained and residual problems • Determine the extent to which there will be a need of future medical treatment and intervention as well as rehabilitation. • Determine the extent to which the person will function in their personal management, leisure, work, and social after having sustained these injuries. • Determine functional and occupational prognosis, vocational potential pre- and post- the accident. • Recommendations with regards to assistance or adaptation in the home environment, assistive devices, and transportation.
